Nominate A Community Champion!

During May’s Older Americans Month we are honoring and celebrating the volunteer efforts of older adults who generously give their time and provide extraordinary volunteer service to better the communities within Palos, Lemont, Orland and Worth Townships. 

 

Our Community Champions will be recognized at a Celebration Luncheon on Thursday, May 7, 2020 at the Orland Chateau, 14500 South La Grange Road, Orland Park. Check in begins at 11:30 A.M. and a family style lunch will be served at 12 Noon. 

Who Is A
Community Champion?

A person from Palos, Lemont, Orland, or Worth Townships, who is 60 years of age or older, whose volunteer service makes or has made a difference in the lives of others. Please do not nominate someone who has been a past honoree. PLOWS Council on Aging and its sponsors of May’s Older Americans Month will provide one complimentary luncheon ticket for every Community Champion. 

How To Nominate A Champion?

Print and fill out the forms hereadd a digital headshot of your nominee and submit all by Tuesday, March 10, 2020.

 

For more information, contact Christine Suddreth at 708-361-0219. Please keep nominations to no more than two (2) Champions per organization, and do not nominate someone who has been a past honoree. PLOWS Council on Aging will contact the Community Champion once we receive your completed form and digital headshot to conduct a brief telephone interview. Please make sure your honoree knows that you nominated them and remind them we will be calling.

NOTE: Please help your nominee secure a digital photo. If they do not have one, contact Christine Suddreth at 708/361-0219 and we’ll arrange for them to have a photo taken at our office in Palos Heights BEFORE the due date.
